Our Company

Foundation One Insurance Services, Ltd., was formed in 1998 in Frisco, Texas, by Managing Directors Keith Appleton and Stephen D. Sims. With over 50 years of combined experience in employee benefits, Mr. Appleton and Mr. Sims recognized the underserved market of uninsured Americans, and the need to provide brokers and consultants with an affordable limited benefit group health insurance program. 

Our Program

Foundation One Security was developed in partnership with American Fidelity Assurance Company, and introduced to a small number of brokers in 1999. The program has since expanded into 41 states, and plan benefits have evolved into today’s broad portfolio of products.

All plan designs provide a wellness benefit, and a combination of other outpatient and inpatient benefits. We can customize a health indemnity plan to suit your needs, and all of our plans can be supplemented with dental, life/ad&d, disability, and vision coverages (where available).

The program is specifically designed for employer groups, and is not intended for associations, unions or contract labor.
